Patients experiencing problems with their bladder or bowels can now use a new digital self-referral tool made available by the County Durham and Darlington NHS Foundation Trust (CDDFT).
The app's goal is to streamline the process of getting patients the assistance they need so they may better manage their disease through self-care.
